Name: List your Eevee's name Age: List your Eevee's age Birthday: List your Eevee's birthday Job: List your Eevee's job (if none, say "none") Main fur color: List the color of fur your Eevee has most of Eye color: List your Eevee's eye color Height: List your Eevee's height (MUST BE REASONABLE FOR AN EEVEE. CANNOT BE MORE THAN TWO FEET TALL.) S.P. (special problems): List any special problems your Eevee may have, like blindness (if none, say "none") Insert your Eevee's picture into the little picture slot. Your card may have special designs on it if desired. The background behind your filled-in Eevee may have special designs on it if desired. Insert animations (ONLY ON FILL-IN) if desired.
