The mod has blood and visible gore, and some of the characters (Sonic.EXE, Lord X, Soul Tails, Soul Knuckles and Soul Robotnik) come from the disturbing source material. The designs of many characters are also quite disturbing. Some characters have poses where they look at the player which can disturb viewers.
