Main weapon here you will put the image of your weapon your going to use Side weapon/armor this is where you put your side weapon or armor Info on weapon This is where you put stuff like weapon stats Character Backstory Where was your character from? Is there anything I need to know about there past? put it here Character 360 this where you will put the front, side and back view of your Oc Character poses These will be poses your character can take. this optional of course and is not required Short character animations This can be little things where you can animate your character to run, use the weapon, etc. it is ofc optional so it is no required This is the legal stuff it clears up anything that may happen to your character. tick or do not tick the boxes according to what you wish That's it! you've read the whole thing (hopefully you didn't just scroll down to the bottom)
