This never happened to me but it can. I have actually found a sunken ship next to a ocean monument. Emeralds, iron, and gold as loot. No diamonds. I always explore in peaceful.
One time i was in the Nether in a no cheat world with achievements. I had my efficiency 3 diamond pickaxe and I was climbing my ladder to come back and i moved sideways and fell in lava. :( I made new diamond pickaxe with unbreaking 3, fortune 2, and efficiency 3. IT IS A AWESOME PICKAXE, I AM SORT OF HAPPY THAT I DIED WITH MY ORIGINAL DIAMOND PICKAXE.
