The twist is that there are more enemies and the game is way harder (just like it says in the title) and you also gain lives every 30 seconds or so. There is also an infinite lives mode if you just want to see the whole thing without having to worry about losing. You can also mute sound effects or music if you want to have it nice and peaceful.
