He has a thing called Nightmare Disorder, which is a disorder where he has frequent nightmares and when awake after the nightmare might experience increased encreased heartbeat and also might be very alert. this might help you understand it better- "During the nightmare, the sleeper may scream and yell out things. The victim is often awakened by these threatening, frightening dreams and can often vividly remember their experience. Upon awakening, the sleeper is unusually alert and oriented within their surroundings, but may have an increased heart rate and symptoms of anxiety, like sweating. They may have trouble falling back to sleep for fear they will experience another nightmare." So that creature is a recurring monster in his dreams. Killing his two friends (shown in the meme) over and over, which he depends on those two friends to be able to live (I will explain it later)
