Controls are operated with the mouse. Click the engine to pull the starter rope. The ignition switch must be in the "ON" position and the throttle lever in the "FAST" position for the engine to start. If the sound "skips" (which it doesn't do much in the Flash player, at least when idling), it's because Scratch is having a hard time processing the scripts.
