made based on a ceiling fan i have in my collection. it is unknown however, i looked up the model on google and couldn't find anything about it. all i know about it is the date of which is 2/29/1995 I will add an image of the fan soon... it's not been working to try and get the image off my phone/ iPad I am struggling right now, please do not expect too much for a few weeks/months
